Travel Vaccinations

The surgery provides a travel advice service which can include provision of the necessary vaccinations if required. Please be advised that not all vaccinations are provided on the NHS and there are charges associated with some types of vaccinations.

For patients requiring travel advice the following procedure applies:

• Prior to any consultation with the Practice Nurses, who administer the service, all patients should complete a pre-travel form. These can be obtained from reception at the surgery, e-mailed to you or printed from this site. Please click here for the pre travel form.

• When the form has been completed please return it to the surgery. 

• Once the form has been assessed by the Practice Nurses you will be contacted to make an appointment or advised that no injections are required.
