PRE-TRAVEL ASSESSMENT

Please fill in and hand a  pre-travel assessment form to a receptionist before making an appointment for travel advice.  We recommend seeking advice at least 8 weeks before travel.  For longer term/gap year travellers, please note that an adequate programme of immunization can take up to 6 months.  Some immunizations are also very expensive. See attached price list.
